Updating Patient records and data

Clinicians need reliable clinical data to provide quality care, case find patients, measure treatment effectiveness and for safety, quality improvement and audits.

Achieving consistent disease coding requires the a whole of team approach A practice policy for clinical coding should address the roles and responsibilitiesfor each team member.
The Australian Digital Health Agency has information on Data Cleansing and Clinical Coding.
ADHA has put together some steps to consider in developing a clinical coding policy for your practice. Also, check out these policy templates developed by the RACGP on secure messaging, clinical coding, ePrescribing and the MHR.
Coding is also important for practices undergoing accreditation to ensure they meet criterions in the Standards for general practices 5th Edition.

Update patient contact details including next of kin and emergency contact, allergy status and ethnicity
Review and update lifestyle risk factors (smoking, alcohol, BMI)

Once imported into your clinical information system, this template will be available to print with pre-populated patient information. The purpose of this template is for practices who may want to update details of existing patients. At the same time as updating information, they can gather data sharing consent. As a privacy risk mitigation strategy it is recommended practices always check patients Medicare card or identity prior to providing these forms to patients The form can also can be amended to be personalised depending on whether practices would like to update some or all details, such as allergy, smoking or alcohol.
